This application may be used for the following reasons: |

To renew a two-year provisional license or a five-year professional or associate license. |

To advance from a two-year provisional license to a five-year professional license. |

To request a correction (ex: name change) to a currently valid license, or to request a duplicate of a currently valid license. |

This application may be used by individuals who are moving from a certificate to a license, and for individuals who are adding a new teaching field at the time of the transition. |

This application may be used by individuals to apply for School Treasurer and School Business Manager licenses, new and renewal. |

|

This application may be used only by employing schools/districts when requesting full-time temporary licenses for teaching, pupil services, administration, and 12-hour teaching permits. This application may not be submitted by individuals. |

|
|
|
|
This application may be used by employing Ohio school districts when requesting educational aide permits and student monitors permits. This application may not be submitted by individuals. |
